I am over the age of 18 and have carefully reviewed and signed the lease agreement. I agree to pay the lease fee for each year of the lease agreement. I understand this lease fee is due whether the team continues to the system or not. I understand that this is a lease and not a sale and that the system must be returned at the end of the lease and that any lost or unreturned parts of the system are subject to a fee as set forth in the lease agreement.
